A male propositus was twin 1 of spontaneous dichorionic diamniotic twins to a non-consanguineous healthy Chinese couple at 36+2 weeks. His antenatal history was unremarkable. His birth weight was 2760 gm (25-50th percentile).1 Height and head circumferences at birth were not documented. Agpar scores were 10 at 1 and 10 minutes. He was noted to have macroglossia and slightly longer fingers at that time. Physical examination of propositus at 3 months of age macroglossia was still present. He also has ear pit on his left ear. Clinical photos of the propositus were shown in Figure 1. Growth curve until 3 months of age found his weight and height were above 97th percentile.
