Abstract Acute lymphoblastic leukaemia commonly presents as fever, pallor and lethargy. Lymph nodes, liver and spleen are the common organs involved. This case report describes a rare presentation of B-lineage lymphoblastic leukaemia infiltrating myocardium leading to heart failure. Leukaemia patients with unexplained heart failure should be investigated for cardiac involvement despite absence of peripheral leukaemia blast.
