We present a patient suffering from Kawasaki disease (KD) with unusual manifestations. Severe myocarditis with shock and gallbladder hydrops dominated the clinical picture in the early phase, when the cardinal features of KD had not yet fully manifested themselves. KD should be included in the differential diagnosis of a febrile child with multi-organ dysfunction.
